Careteam aims to unite patients and healthcare providers with a platform approach

How best to untangle the Gordian knot that is navigating your own healthcare? It’s a tricky question, and one that seems to have become only more complicated as technology improves, in many regards — systems don’t necessarily speak to one another, and it’s still hard for an ordinary patient without specialist knowledge to make sense of everything. Careteam is a Canadian startup hoping to address that, looking to replicate the kind of advances made possible by technology in industries like e-commerce and enterprise software.

Careteam co-founder and CEO Dr. Alexandra Greenhill has experienced the frustration of being a tech-savvy person in a world of healthcare that can seem technologically inept — both as a practicing GP and as someone who depends on the healthcare system as a patient and a relative of patients with more sophisticated medical needs.

“I spent more than 15 years innovating within the healthcare system,” Greenhill told me in an interview. “I computerized hospitals, helped doctors adopt electronic medical records and other types of innovation practices. And then for the last eight years, I’ve been in tech, trying to figure out how to build the kind of technology we need in health, and especially digital health.”

All that experience led Greenhill to the realization that while there were many companies building specific solutions for real, but relatively narrow problems, that didn’t reflect how most people experienced care. Greenhill and her team of three other co-founders (Jeremy P. Smith, Robert I. Atwell and Kevin Lysyk) had all had unfortunate, but eye-opening experiences with family members in need of treatment for major diseases.

“You step in and you discover that cancer care, palliative care, post-surgical care — there’s so many things that would have gone wrong if we didn’t have the expertise ourselves,” Greenhill said. “But in the meantime, you end up being sort of pulled into multiple directions and saying ‘this makes no sense.’ You know, I can purchase stuff online in my private life; I can use all kinds of tools in the business world, and yet it’s back to paper and voice in health, which matters most.”

What Careteam provides is collaboration for care — true collaboration, designed to span patients, their doctors and other healthcare pros, their families and anyone who matters to them in the course of pursuing their care. It provides the ability to communicate instantly, build care plans that integrate all aspects of their tailored health plans, receive custom-configurable notifications and measure progress toward specific goals set by patient and healthcare providers.

Part of the reason this process has become opaque or difficult is precisely due to innovation: Greenhill takes issue with the prevailing narrative that the healthcare industry is somehow allergic to innovation.

“There’s this sort of perception that healthcare doesn’t innovate, but it’s also almost insulting to the healthcare system, because we have innovated — we save people from cancer, where we couldn’t,” she noted. “We cure HIV, in some cases, and we prevent it from being transmitted to unborn babies of mothers with full-blown AIDS and things that in my working lifetime were impossibilities; it was science fiction to help someone with HIV. And, and we’ve managed to do all of that, and it’s a success story. We’ve created complexity, we’ve created people who live with 12 conditions for many, many years and take complicated drug regiments.”

In addition to advances in treatment, Greenhill notes that she and her team couldn’t have build Careteam five years ago, because cloud storage wasn’t secure and everything had to be done on a site-specific instance, and that would’ve been cost-prohibitive to build. In other words, technology has been applied to, and vastly improved, healthcare overall, regardless of the general perception of the industry as an innovation laggard.

That’s why Greenhill’s startup doesn’t shy away from complexity — they embrace it. Careteam is designed not to try to normalize and standardize the varied and highly specialized landscape of healthcare solutions and providers through anything like a one-size-fits-all API. Instead, the company’s tech development is cleverly designed to be flexible when it comes to integrations.

“We collectively spent $1.9 billion in Canada, to try and digitize the healthcare system, create standards and create some exchange between data,” Greenhill said. “The NHS tried the same, big U.S. hospital systems have created their own little sort of islands, including Kaiser and Mayo and others. And the conclusion of all of that is standardization in healthcare just doesn’t seem to catch on.”

Careteam’s approach has been instead to integrate specific clinics, and let practitioners and patients derive benefits and help spur the adoption of the platform to their companion organizations and clinics. It’s a sort of rhizomatic approach that starts with a node central to a patient’s care and spreads through the healthcare professionals and members of the patient’s support network that the product helps. And integration is made possible without technical demands on the part of partners thanks to the work of CTO Lysyk, according to Greenhill.

The Vancouver-based startup is working with the Centre for Aging + Brain Health in Toronto, Ontario in a validation program announced last year, and also raised an initial round of funding in January led by BCF Ventures with participation from Right Side Capital, Globalive Capital, Atrium Ventures, and angels Barney Pell and Ajay Agarwal .

Jio Health combines online and offline healthcare in Southeast Asia, starting in Vietnam

The internet is often lauded for the potential to increase the impact of a range of primary services in emerging markets, including education, commerce, banking and healthcare. While many of those platforms are now being built, a few are finding that a hybrid approach combining online and offline is advantageous.

That’s exactly what Jio Health, a “full stack” (forgive the phrase) healthcare startup is bringing to consumers in Southeast Asia, starting in Vietnam.

The company started as a U.S.-based venture that worked with healthcare providers around the “Obamacare” initiative, before sensing the opportunity overseas and relocating to Vietnam, the Southeast Asian market of 95 million people and a fast-growing young population.

Today, it operates an online healthcare app and a physical facility in Saigon; it also has licenses for prescriptions and over the counter drug sales. The serviced launched nearly a year ago; already the company has some 130 staff, including 70 caregivers — including doctors — and a tech team of 30.

The idea is to offer services digitally, but also provide a physical location for when it is needed. Therein, the company ensures that “every element of that journey” is controlled and of the required standard; that’s in contrast to services that partner with hospitals or other care centers.

The scope of Jio Health’s services range from pediatrics to primary care, chronic disease management and ancillary services, which will soon cover areas like eye care, dermatology and cancer.

“Our initial research [before moving] found that healthcare in Vietnam was unlike the U.S.,” Raghu Rai, founder and CEO of Jio Health, told TechCrunch in an interview. “Spending is primarily driven by the consumer (out of pocket) and there’s no real digital infrastructure to speak of.”

Rai — a U.S. citizen — said doctors typically “have minutes per patient” and get through “hundreds” of consultations in every morning shift. That gave him an idea to make things more efficient.

“We can probably address north of 80 percent of consumers’ health needs,” he said of Jio Health,” but we also have referral partnerships with certain hospitals.”

The process begins when a consumer downloads the Jio Health app and inputs primary information. A representative is then dispatched to visit the consumer in person, potentially within “hours” of the submission of information, according to Rai.

He believes that Jio Health can save its users money and time by using remote consultancy for many diagnoses. The company also works with health insurance companies for areas like annual checkups, and Rai said that McDonald’s and 7-Eleven are among the corporations that offer Jio Health among the providers for their staff; they’re not exclusive.

This week, Jio Health announced that it has closed a $5 million Series A funding from Southeast Asia’s Monk’s Hill Ventures . Rai said the company plans to use the capital for expansion. In particular, he said, the company is adding new care categories this month — including eye care and dermatology — and it is working toward expanding its brand through marketing.

Further down the line, Rai said the company hopes to expand to Hanoi before the end of this year. While there is interest in moving into other markets within Southeast Asia, that isn’t about to happen soon.

“We have begun to investigate other markets, but at this point feel the market in Vietnam is substantial in itself,” he told TechCrunch. “It’s very plausible that we’d be looking at international expansion plans in 2020… we’re going to be focused on Southeast Asia.”

YourChoice Therapeutics is developing unisex, non-hormonal birth control

The options available to women who want to avoid getting pregnant today are bad. Most, like the widely used birth control pill, feed man-made estrogen and progestin hormones to women, which are capable of causing a number of awful side effects.

YourChoice Therapeutics — a startup launched by a team of Berkeley researchers, including two experts in sperm physiology and sperm-egg interactions — dreams of producing a unisex, non-hormonal alternative to existing contraceptives. The company has raised $400,000 in funding to date, plus a $150,000 check from Y Combinator. YourChoice will make its big pitch at Y Combinator Demo Days next week.

It’s seeking $2 million in venture capital funding to continue research on its sperm cell-targeting novel method of contraception, as well as to build out its team of chemists. Founders Akash Bakshi and Nadja Mannowetz tell TechCrunch they plan to have a contraceptive ready to market by 2025. Together, with co-founder and advisor Dr. Polina V. Lishko of Berkeley’s department of cell and molecular biology, they hope to reach women and men all over the world, in the process tapping a market expected to be worth $37 billion by 2023.

“There are perhaps ways that we could cut that time in half or just get something to market,” said Bakshi, YourChoice’s chief executive officer, whose background is in technology commercialization, research and development within the life sciences industry. “But we need to do this right so that we can benefit as many women as possible.”

Their first product will be a vaginal contraceptive to be applied before intercourse, then, the startup plans to release oral contraceptives for both genders. The team has discovered that the natural compound lupeol is capable of blocking a protein on sperm that is required for fertilization. YourChoice‘s non-hormonal approach doesn’t impact a cells’ ability to function or gene expression, so women and men are not at an increased risk of blood clots, cancer or other side effects associated with mainstream birth control methods’ use of added hormones.

“The bottom line is men don’t have good options and women apparently have so many choices, yet they are all really bad,” Mannowetz, a Ph.D. in sperm physiology, told TechCrunch. “They’re all based on that over 60-year-old idea of hormone-based drugs.”

YourChoice’s planned debut product will be applied directly in the vagina during the period of the month in which the woman is fertile. Whether that be a tablet, a gel or some other form factor is still up in the air. YourChoice’s second product will be an oral contraceptive because they believe that is the most convenient, universally accepted method.

“For women who have an implant … I understand that this might be a step backward, but women who have been on the pill for decades, for them, it wouldn’t be a big change,” Mannowetz said. “We totally understand we will not serve every woman out there but we need to get started with a product and then take it from there.”

“If the last 60 years have taught us anything, it’s that delivery is something that can continue to be developed,” she continued. “We need to develop a new mode of action.”

There are a number of startups innovating in the contraception space, as TechCrunch has written, though most of those businesses are focused on the access problem. Birth control can be very difficult for many to access and startups like The Pill Club or Nurx solve that problem by delivering the pill directly to women’s doorsteps. Other early-stage companies in the space lack experts in the field of reproductive biology necessary to improve contraceptive options. YourChoice’s team says seeking change to the actual medication with an advanced team sets them apart from other upstarts.

For YourChoice, it helps that venture capital investment in the reproductive tech space is increasing, making this a great time for YC to support these businesses (YourChoice isn’t the only reproductive tech startup in the latest YC cohort) and for YourChoice to successfully nab private investment.

“I personally think the industry is satisfied; they are making really good money, right? So why should they change anything,” Mannowetz said. “Millennials are the starting point of change happening. I think now, women stand up and say, ‘we are sick of it.’ ”

Lack of transparency in healthcare startups risks another Theranos implosion

Are more Theranos -style scandals looming for investors in healthcare startups?

A team of researchers associated with the Meta-Research Innovation Center at Stanford thinks so. They’ve published a paper warning investors in life sciences startups that a systemic lack of transparency exists in their portfolio companies — creating the possibility for more multi-billion-dollar implosions and scandals like the one that toppled Theranos and its charismatic founder, Elizabeth Holmes.

Indeed, one of the study’s authors, Dr. John Ioannidis, the co-director of the Meta-Research Innovation Center at Stanford and director of the University’s PhD program in Epidemiology and Clinical Research, was  among the first people to identify the risks associated with Theranos and its “stealth research.”

Now Dr. Ioannidis and his co-authors, Ioana A. Cristea and Eli M. Cahan, have published a study surveying the publicly available research from the largest privately held companies in the healthcare space, and found them lacking. 

Most of the highest-valued startups in healthcare have not published any significant scientific literature, the study found. Nearly half of the publications from companies worth more than $1 billion came from only two startups — 23andMe and Adaptive Biotechnologies, according to the paper.

“Many years ago I was the first person to say that Theranos had a problem,” says Ioannidis. “The problem that I had then was that Theranos did not have any peer-reviewed evidence to show.”

In an interview and in their paper, Ioannidis and Cahan warn that investors have overlooked systemic problems created by the lack of transparency among healthcare startups.

They write:

It would be tempting to dismiss the Theranos case as just one rotten apple. However, we worry that the focus on fraud puts aside a more fundamental concern. Fraud is making waves in the news, but stealth research may have a more detrimental impact.

According to the study’s findings, more than half of the healthcare startups that are worth more than $1 billion have published no highly cited papers at all. For companies that were acquired or are publicly traded that number is around 40 percent.

In all, healthcare startups that are currently valued at more than $1 billion published 425 Pubmed papers. And of those papers only 34 (8 percent, including two reviews) were highly cited. For companies with valuations of more than $1 billion that had been acquired or are publicly traded on stock exchanges, the researchers counted 413 papers, of which 47 (11 percent, including nine reviews) were highly cited.

Digging deeper into some of the companies that had high valuations but little or no published research revealed scores of operational and technological issues for the researchers.

For instance, StemCentrx, which was bought for $10.2 billion in 2016 by AbbVie, had published 16 papers — and only one highly cited paper. Since the acquisition, the Food and Drug Administration had imposed a delay on the readout of the company’s phase II trial for its Rova T targeted antibody drug for cancer treatment. In December, a Phase III trial for Rova T as a second-line treatment for patients with advanced small cell lung cancer was halted because the treatment wasn’t working, according to a report in Targeted Oncology.

Acerta Pharma, another healthcare-focused startup focused on cancer treatments, was bought by AstraZeneca for $7.3 billion. That company published nine articles and had one highly cited paper for a very early study of a potential treatment for relapsed chronic lymphocytic leukemia. Acerta received accelerated approval for a drug called acalabrutinib, which treats a rare form of lymphoma called mantle cell lymphoma. Two years ago, AstraZeneca had to retract data and admit that Acerta falsified preclinical data for its drug.

Then there’s Intarcia, the developer of a device for diabetes treatment that’s worth $5.5 billion. That company had its device rejected by the FDA and was forced to lay off staff and halt a couple of later-stage trials. It had only published six papers — none of them very highly cited.

Ultimately, the researchers concluded that highly valued healthcare startups don’t contribute to published research and that the valuation of these companies by investors is divorced from any externally validated data.

For the researchers (and for investors) this should present a problem.

“Many unicorns may be overvalued [21] and subject to unrealistic scientific expectations,” the study’s authors write. And they reject the argument that simply applying for — and receiving — patents is enough to prove that a technology in the healthcare space has been thoroughly vetted. “[Patents] do not offer the same level of documentation as peer-reviewed articles. For example, Theranos had over 100 patents [1], but these were unable to supplant the vacuum in their evidence,” the researchers wrote. 

Even if companies want to protect their technology, there are still ways for them to be more transparent about the results or benefits of their technology. The authors acknowledge that publishing isn’t the primary mission of startups. They can, however publish a few high-value articles, secure their technology through patents and then work with researchers, universities or hospitals to validate the technology and have those organizations publish results of the tests, the authors argue.

As the authors conclude:

Start-ups are key purveyors of innovation and disruption. Consequently, holding them to a minimal standard of evaluation from the scientific community is crucial. Participation in peer review, with all its limitations, is the best way we have to uphold this standard. We are not arguing that start-ups should divert excessive resources to having peer-reviewed papers. However, when their products are destined to affect patient health, they should neither be solely doing marketing. Confidential data sharing with potential investors or regulators cannot replace more open scrutiny by the scientific community.

As biological manufacturing moves to the mainstream, Synvitrobio rebrands and raises cash

The pace at which the scientific breakthroughs working to bend the machinery of life to the whims of manufacturing have transformed into real businesses has intensified competition in the biomanufacturing market.

That’s just one reason why Synvitrobio is rebranding as it takes on $2.6 million in new financing to pursue opportunities in biopharmaceutical and biochemical manufacturing. Under its new name, Tierra Biosciences, the company hopes to emphasize its focus on agricultural and biochemical products.

The company is one of several looking to commercialize the field of “cell-free” manufacturing — where biological engineers strip down the cellular building blocks of life to their most basic components to create processes that ideally can be more easily manipulated to produce different kinds of chemicals.

There’s a standard way to create these cell-free processes (described quite nicely in The Economist).

Grab a few quarts of culture with some kind of bacteria, plant or animal cells in it. Then use pressure to force the cells through a valve to break up their membranes and DNA. Give the goo a nice warm environment heated to roughly the average temperature of a human body for about an hour. That activates enzymes that will eat the existing DNA.

Put all of it in a centrifuge to separate out the ribosomes (which are the important bits). Take those ribosomes and give them a mixture of sugars, amino acids, adenosine triphosphate (the molecular compound that breaks down to provide energy for all biological functions) and new DNA with a different set of instructions on what to make and voila! Micro-factories in a test tube.

Along with co-founders Richard Murray of the California Institute of Technology and George Church, one of the living legends of modern genetics, chief executive officer Zachary Sun designed Tierra to be an engine for new biochemical discovery.

“Everything floats in the cytoplasm… We keep that internal stuff and that allows us to run reactions where a cell wall isn’t necessary. I want to reduce the complex system down to its component parts,” says Sun. “We look at this as a data collection problem. We want to use cell-free to tell you what to put either in a cell or in cell-free systems… We can collect more data faster using our cell-free system.”

The startup is already working with the Department of Energy research institution at Oak Ridge National Laboratory to develop processes to create vanillin (vanilla extract) and mevalonate (turpentine) from biomass.

It’s an approach that is already showing the potential for investment returns in life sciences and pharmaceuticals. For inspiration, Tierra can look to the South San Francisco-based Sutro Biopharma.

That company has signed a drug discovery agreement with Merck to develop new immune-modulating therapies (that bring the immune system into check) for cancer and auto-immune disorders, in a deal worth up to $1.6 billion if the company hits certain milestones — in addition to a $60 million upfront payment. Sutro raised more than $85 million in new funding in July (from investors including Merck) and just filed to go public on the Nasdaq.

According to Sun, the newly named Tierra has its own partnerships with global 2,000 companies in the works. “We’re looking to scale those commitments. We see the application space as being this natural products environment,” he says.

There’re multiple avenues to pursue, with the technology widely applicable to everything from pesticides to pharmaceuticals, flavorings and even energy.

“Synthetic biology at its core is about applying engineering best practices to speed up the ‘design-build-test’ cycles in the reprogramming of existing or construction of new biological systems. By component-izing and modularizing the cell they can radically increase the speed of those cycles,” says Seth Bannon, a co-founder of the venture capital firm Fifty Years, which invests in startups commercializing “frontier” science. 

For the investors, entrepreneurs and reporters who witnessed the birth of the cleantech bubble a decade ago and then tracked its implosion in subsequent years, the excitement this kind of technology elicits is another of history’s rhymes.

Technologies like Tierra’s aren’t new. San Diego-based Genomatica has been working on biological manufacturing for the past 18 years. The company is now exploring a cell-free system to grow chemicals that are used in the manufacture of materials like Lycra. Since 2008, Medford, Mass.-based GreenLight Biosciences has been working to bring its own biologically based zero-calorie sugar substitute to market.

What may be different now is the maturity of the technologies that are being commercialized and the perspective of the startups coming to market — who have the benefit of avoiding the missteps made by an earlier generation.

Investors led by Social Capital with participation from Fifty Years, KdT Ventures and angel investors seem to see a difference in these companies. And large research institutions are also marshaling resources to support the vision laid out by Sun, Murray and Church. DARPA, the National Institutes of Health, the Department of Energy, Cyclotron Road and Lawrence Berkeley National Laboratory, the National Science Foundation and the Gates Foundation have all backed the company, as well.

“So many therapeutic molecules come from nature. As the DNA of plants, animals and microbes is read in exponentially increasing volume, we expect to find useful and game-changing chemistry encoded by it. Tierra’s platform will allow us to look for molecules which might otherwise be buried in the complexity of cells’ metabolism,” says Louis Metzger, chief scientific officer of Tierra, who comes from a background of drug discovery.

Overwatch ‘Pink Mercy’ sale raises $12M for breast cancer research

It can be hard sometimes to grok the scale of the gaming community, but the occasional charity event not only demonstrates the hugeness of the industry but also its diversity and willingness to shell out for a good cause. Today Blizzard announced that an Overwatch charity campaign raised an impressive $12.7 million for breast cancer research in just two weeks.

Overwatch is an extremely popular team-based shooter game that has made an impression not just with its solid gameplay, but its striking and inclusive character design. This sensitivity to the ever-widening demographics of gaming led them to conceive of this charity campaign back in May.

Players could for a limited time purchase a special “skin,” or 3D model, for the character Mercy — she’s the most powerful healer in the lineup, so the choice makes sense, even though the statuesque blonde isn’t exactly their most interesting character work. (A Pink Genji would probably look cool, but it would probably just make more people play him — a regrettable outcome.)

Special skins are highly sought-after, and while many can be obtained through in-game loot boxes, they can also be purchased. In this case, the price was set at $15, rather high for a skin but clearly that didn’t deter players, who shelled out by the thousands for both it and related t-shirts.

I asked for a breakdown, but a little napkin math gives a basic idea of the volume. The press release announcing the $12.7 million number says “thousands” of t-shirts were sold at $30 apiece; usually if it’s 10,000 or more they say so, so we’ll just use 10K as our estimate. That makes $300K from shirts, so the remaining $12.4 million means somewhere north of 820,000 people paid for the Pink Mercy skin.

Think about that! In two weeks more than three quarters of a million people paid $15 each for a virtual item. Pretty great. It’s all going to the Breast Cancer Research Foundation, by the way. They got a big novelty check:

And this is by no means the only big gaming charity event. Games Done Quick regularly raises millions, and Penny Arcade’s Child’s Play got so big that it had to be spun off as its own thing. It just recently announced a round of grants funding pediatric hospital equipment and staff, by the way.

This event went well enough that we can probably expect more in the future for other causes — I’ve asked Blizzard for any details on that front and will update if I hear back.

Grail is raising at least $1 billion to fund its early cancer screening test
